wiki: refactor and cleanup (#57) 
- tidy up traefik middlewares
- mount db to `/storage`, remove constraint on single host
- separate `LocalSettings.php` into its own file for sanity
- move wiki-related files to `wiki/`
- use traditional wiki skin by default
